We may also collect gender, NIN, NHS number and other information about your dental and general health, including clinical records made by other relevant medical professionals involved with your care and treatment.
We also provide an online portal where other medical professionals can use our online referral form. To complete the referral form patient details may be required including title, name, address, email, telephone, reasons for referral and relevant medical history.
